The currently used treatment options for this condition are suboptimal since the pathogenesis remains unclear. As addressing conventional pathogenic mechanisms of uncontrolled hyperglycemia, hypertension and RAAS activation didn’t halt the progression of DN, exploring additional molecular mechanisms became necessary.
Significant advances have occurred in the pathogenic role of growth factors, vasoactive factors such as nitric oxide and endothelins, and cytokines such as NF-kB and interleukins is being explored. Current use of experimental techniques such as Next Gen sequencing has expanded our knowledge of the pathogenic factors including genetic, epigenetics of DN.
Development of suitable animal models such as ZSF rats have increased our understanding of the pathophysiology and facilitated the investigation of potential novel therapies.
